725 Paediatric Resuscitation Burns Maintenance Fluid Selection - a Retrospective Review of Cases and Adherence to NICE, LSEBN and EMSB Guidelines

Abstract Aim Appropriate fluid management in major burn injuries is directly related to survival rates. Despite this fact, there are still controversies regarding the choice of replacement given during resuscitation and maintenance stages acute burns. The aims study compare current paediatric prescriptions against guidelines, assess whether any adverse effects (i.e., electrolyte disturbance). Method Details all burn's patients (aged 18 years or under) who were admitted burns ward an 18-month-period screened. Only those coded as having sustained 10% TBSA greater included data analysis. Data was then sourced using Cerner electronic patient record software. Results A total 17 identified, sustaining scald with between 22% Every 0.45% NaCl 5% dextrose fluid. All IV supplementation ceased after 24hrs. 53% had pre- post-fluid bloods taken. Average drop sodium amongst these 2mmol, largest (5mmol) % TBSA. Conclusions We found zero compliance NICE which suggests isotonic crystalloid (such 0.9% dextrose) for maintenance, therefore guidance line should be considered.
